This site contains scanned images of the cover of the application for payment of the accrued pension. Pages were scanned at 300 dpi resolution to give the greatest detail for printouts.

The ledger sized outer jacket was folded into four parts. Reproduced below is the single exterior surface which had writing.

The inside of the cover has a large bottom margin, with writing crossing the upper two thirds of the sheet. Writing is perpendicular to the writing on the opposite side. To reduce the size of the image, blank portions were cropped.
