Letter: "[Form] 3-151. Page 98-33 Med. Div. Department of the Interior. MAD BUREAU OF PENSIONS, Washington, D.C., July 16, 1907. Board of U.S. Examining Surgeons, Vienna, Johnson Co., Ill. Drover's Bank Building. Dr. W.R. Mizell, Pres. Gentlemen: In the case of John Brandon, Co. G, 31 Ill. Vol. Inf., Ctf. No. 79,142, whom you examined on June 19, 1907, further information is desired as indicated below. Please write your amendment upon the accompanying blank and forward it promptly in the inclosed (sic) envelope, together with this letter. It should be dated, and signed by each member who participated in the examination. If it is necessary to recall the applicant, he may be addressed at Stone Fort, Saline Co., Ill. Relative to gunshot wound of left side, your attention was called to soldier's contention that the missle also injured the spine, but no reference to such injury is found in the report. Please state whether there are any physical signs or any subjective or rational symptoms indicating injury either to the spinal column or to the cord. Is there deformity or displacement of a vertebra or curvature of the spine? Does pressure cause pain or tenderness or any abnormal sensation? Does he carry one shoulder higher than the other? Is there evidence of spinal irritation - hyperasthesia or parasthesia at any point below the level of the wound? Note condition of tendon reflexes. Any incoordination of muscular movement? Request for Pension Increase: "DECLARATION FOR INCREASE OF PENSION, GENERAL LAW. State of Illinois, County of Saline, SS: ON THIS 9th day of May, A.D. 1908, before me a Notary Public, in and for the County and State aforesaid, personally appeared John Brandon, aged 63 years, a resident of Stonefort, County of Saline, State of Illinois, who being duly sworn according to law, upon his oath declares that he is a pensioner of the United States, duly enrolled at the pension agency at Chicago Illinois at the rate of $30 Dollars per month, by reason of disability incurred in the military service of the United States while a Private in Co. "G" 31st Reg.t Ill. Vols Infty. That the disability for which he draws pension is for gunshot wound of left side and lugs and results and that his pensioned certificate is number 79.142 that his present physical condition is such that he believes himself entitled to a increased pension, and that he therefore makes this application for such increase for the following reasons: that he is totally disable for the performance of manual labor of any kind or character. That he is not now able to care for his person. Cannot dress nor undress himself. That he is now compelled to have the constant aid and assistance of other persons. All of which is caused from the above named disabilities. That he is not now able and never expects to be able to travel and go any where for examination. And therefore request that he be examined at his home and at the earliest possible moment. He hereby appoints, with full power of substitution and revocation, himself of the County of Saline, State of Illinois as true and lawful attorney to prosecute this claim. Medical Affidavit: "PHYSICIAN'S AFFIDAVIT, TAKE NOTICE. - The affidavit should, if possible, be in the handwriting of the affiant. All the facts in the possession of affiant as to the origin and continuance of the disability should be fully set forth, and the dates of treatment should be specifically given. If the affidavit is prepared from memoranda in possession of the physician, that fact should be stated. State of Illinois. County of Saline, SS: In the Matter of increase pension claim of John Brandon, of Co. "G" 31st Regt Ill Vols Infty. No. 79,142. ON THIS 9th day of May, A.D. one thousand nine hundred and Eight, personally appeared before me, a Notary Public within and for the County and State aforesaid, duly authorized to administer oaths, Dr. B.F. St. John, M.D., aged 73 years, a resident of No. _ Street, Stone Fort, County of Saline, State of Illinois, well known by me to be reputable and entitled to credit, and who, being duly sworn according to law, declares, in relation to the aforesaid case as follows: THAT he is practicing physician and has been acquainted with said John Brandon for about 25 years, and that he has the means of knowing and does know from frequent examinations in the past, that said soldier has been suffering from gunshot would of the left side and lung during which time he has frequently treated him and gave him medical advice. That said soldier has never been able to perform manual labor of any kind, that he has had a continuous cough ever since he has known him. Has often prescribed cough remedies for him. That he has this day made a profesional examination of said solder and find him suffering from said injuries. The lung effiction predominately, that soldiers condition is now such that he is compelled to have the aid and assistance of a regular attendant. That soldier's condition is now such that he cannot leave his home for examination before any ex board. And will give as my opinion that he never will be, as considers his time on earth, but very short. That he reccommends that said soldier examined at his home at the earliest concurrent moment for the reason that his case merits immediate action. He further declares that he has been a practitioner for 48 years, and that he is not interested in the prosecution of this claim. His post office address is Stone Fort, Saline County, State of Ill. Swan, Secretary, Harrisburg, Co. Saline, (State) Ill. Sir: Mr. John Brandon, P.O. Stonefort, Co. Saline, (State) Illinois, late a Private, Co. G, 31 Regiment, Ill. Vol. Inf. an applicant for Increase. Invalid Pension Crf. No. 79.142. for examination on account of the disability or disabilities mentioned hereon. If other causes of disability are found on examination, they should be described. Evidences of the results of vicious habits should be sought in every case and reported on. Letter requesting Exam: "HOME. It is the practice of the Bureau not to order a surgeon to travel over thrity miles from his home to make an examination. If it is desired that a surgeon should travel a greater distance than thirty miles, that fact will appear in this order. If it is not clearly set forth in the order to the surgeon, he should notify this Bureau, that the order may be canceled, or the surgeon instructed to comply with its requirements. [Form] 3-133. As soon as possible, therefore, at a time which will not conflict with a meeting of your board, the member of your board residing nearest to the above will proceed to the home of said claimant at Stonefort, Saline Co., Ill. and examine him very carefully in accordance with the instructions inclosed, and then return to the Bureau this letter and the other papers herewith, closely observing the directions in Form 3- 168. SIR: You are informed that an order has this day been issued, directing a member of the board of United States examining surgeons at Harrisburg, Ill. to examine you at your home with reference to your claim for pension. The doctor should write the date of his examination in the space below, and you should return this notice to the Bureau. Very respectfully, V. Warren, Commissioner. Medical Examination: "[Form] 3-155. SURGEON'S CERTIFICATE. Increase, Pension Claim No. 79142. Name of claimant, John Brandon. Company G, 31 Reg't Ills. Vol. Inft. Claimant's post office address, Stone Fort, Saline Co., Ills. Address of Board.{ Harrisburg P.O., Illinois, State. June 3rd, 1908 (Date of examination). Names of disabilities, gunshot wound of left side & lung. He receives a pension of 30 dollars per month. He makes the following statement in regard to the origin of his disabilities and date when first discovered by him: Received G.S.W. 16 day of May 1863 at the battle of Champion Hill Miss. Birthplace, Roane Co., Tenn; age 63 years; height, 5- 7 inch; weight, 109 pounds; complexion, light; color of eyes, blue; color of hair, gray; occupation, none; permanent marks and scars other than those described below, none. We hereby certify that upon examination we find the following objective conditions: Pulse rate, 76, 82, 99 (sitting, standing, after exercise.); respiration, 26, 31, 39 (sitting, standing, after exercise.); temperature, 99; muscles very soft & flabby much mal- nutrition. Skin sallow - he is scarcely able to walk alone he is so poor. G.S.W. of left side. There is a scar _ inch by 1 1/2 inches long in the left axillary region a little above a line down a round the body from the nipple to nipple & 6 inches from the nipple. Place of entrance scar neither dragging nor adherent. And another scar bout 1 inch in diameter just to the right side of the spine (bout 1 1/2 inches and 2 1/2 inches above line described, scar slightly adherent point of exit. Missil (sic) passing through the lung and over the spine taking off the spnons process of one of the dorsal vertebrae. Causing tenderness along course of spine and adjacent muscles on pressure. Claims that pains are very severe at times in the back and the muscles are so soft that he has to have help to dress & undress all the time. Rate 17/18 Lungs (G.S.W.) Measurement of chect Inspiration 32 1/2 inches Espiration 31 inches - Bilateral measurement shows right side 1 1/2 inches larger that left and in fact left side does not sispond but very little, hardly perceptible. Vocal formitus slightly incrasid over right lung and pronounced over left purcussion, Resonaced dull over over over (double word, sic) left lung & slightly impaired over right. Viscular murmer intirely absent in luft lung scarcely any air passing in and lung & Visiculor murmur slightly limpassed in right. He has harassing cough & exprectrates puss & often tinerd with blood. He claims that on change of weather his cough is worse and that it has to sit up or leave his head elevated of nights to get any rest at all. Rate 17/18. Heart ahip (?) evidued (?) to inspection & palpation in 6th interspace in mamillary line dullness bout 4 x 6 inches. Rhythum slightly dysturbed (sic) Feeble action but very rapid on slight exercise. No murmers some dysproval & cyonosis but no Iaimia. Rate ?. S.J. Blackman, Treas."
Addendum to Examination: "[Form] 3-156. SURGEON'S CERTIFICATE. For use when additional space is needed to complete or amend report of examination. Pension Claim No. 79142. Name of claimant, John Brandon, Company G 31, Reg't. Ills Vol Inft. Address of Board{ Harrisburg, Ills. (date of examination) June 3, 1908. EXAMINATION - Continued. Kidneys Spec. 1017, Color amber. Reaction action, no albumen. No sugar, Rate O. His emaciated feeble condition due to pension cause alone Is what confines him to his home. It is my opinion that the claimant is disabled in a degree that requires the foreground and practical aid and attendance of another person due to G.S.W. of left side & lung and is intitled to $50.00 per month. Increase Rejected: "[Form] 3-427. Western Div. M.M.J., Ex'r. Certificate No. 79142 Department of the Interior, John Brandon, G. 31, Illinois Inf. BUREAU OF PENSIONS, Washington, D.C. July 27, 1908. John Brandon, Stone Fort, Illinois. Sir: You are informed that the above- entitled claim for increase of pension under the general law is rejected on the ground that the evidence on file shows that a rate of pension in excess of $30 per month is not warranted by the degree of disability from pensioned cause gun shot wound of left side. Very respectfully, V. Warren, Commissioner."
Appeal for Increase: "STATE OF ILLINOIS, SALINE COUNTY,} SS. IN THE MATTER OF Appeal to Secy of Interior for pension claim increase cert. No. 79,142 of John Brandon, Co. G, 31st Regt Ill, Vol Inf. Personally came before me, a Notary Public in and for the aforesaid County and State, John H. Denny aged 45 years, a citizen of Stonefort of Saline, State of Illinois well known to me to be reputable and entitled to credit, and who being duly sworn declares in relation to the aforesaid case as follows: That he is acquainted with John Brandon the above named Soldier and has been personally acquainted with him for fifteen years last past. That he lives a near neighbor to him and has since his acquaintance, that he frequently visits him and that he has knowledge of his condition as to health. Said affiant further deposes and says that from this acquaintance and knowledge he knows the said soldier to be totally disabled and he saw the said Soldier John Brandon does require and keeps constantly an attendant and attendants and it - is his belief that the disability of said soldier was caused by gun shot wound of left side resulting in diseat of his lung which wound was received while in the service of the United States as such soldier he further declares that he has every reason to believe that the said soldier John Brandon will never recover. He further declares that he has no interest in said case, and is not concerned in it's prosecution. John H. Denny. [no witnesses signed]. Dunn aged sixty six years, a resident of No. _ Street, Stonefort, County of Saline, State of Illinois and Harry Gleyr aged thirty one years, a resident of No. _ Street, Stonefort, County of Saline, State of Illinois well known by me to be reputable and entitled to credit, and who, being duly sworn, declare, in relation to the aforesaid case as follows: That they and each of them are well acquainted with John Brandon, the above named soldier and have been so acquainted for many years last past. That they are well acquainted with his past and present physical condition. That he has been suffering for many years last past from what they understand to be disease of lungs as result of gunshot wound through lung and body. That they and each of them live near neighbors to said soldier and make frequent visits to see and ascertain his condition. That they have the means of knowing and do know that from his enfeebled condition in consequence of said disease he is confined to his bed and room and is now compelled to have the constant (not perivdreal)(?) and and (sic) assistance of other persons both day and night. Claimant's Appeal: "CLAIMANT'S APPEAL TO THE SECRETARY OF THE INTERIOR. State of Illinois, County of Saline, SS: On this fourth day of August, A.D. one thousand nine hundred and Eight, in the matter of claim for increase of pension No. 79.142, John Brandon Claimant. Personally came before me a Notary Public, in and for the aforesaid County and State, John Brandon, aged 63 years, late Private, Co. "G", 31st Reg't Ill. Inft. Vols., a citizen of Stonefort, County of Saline, State of Illinois, well known by me to be reputable and entitled to credit, and who being duly sworn, declares in relation to the aforesaid case as follows: That his claim for increase Pension No. 79.142 having been rejected for the follow reasons. "That the rate of pension increase of $30 per month is not warranted by the degree of disability from pensioned causes, gunshot wound of left side" and that the extent of his disabilities are total. That he is confined to his room and bed and is compelled to have the constant and cons. assistance of other persons both day and night. All in consequence of disease of lungs resulting from gunshot wound, and for which disease and injury he is now pensioned. The Claimant appeals for a Reconsideration of his said claim for the reason that great injustice has been him by the Hon. Commissioner of pensions in the rejection of his sent claim. That his condition was such at the time he filed his application for increase which will must fully appear from the affidavit of Dr. B.F. St. John ? on file in the pension office, which is procured to be considered as part of this application for reconsideration that his condition was such as to personal the and of other persons at the time he was examined by Dr. S.J. That therefore requests that the Honorable Secretary of Interior may reopen and reconsider his claim and allow him an increase of his pension consomerate with his disabilities. Clitwood aged 68 years, a resident of No. _ Street, County of Johnson, State of Illinois, and . well known by me to be reputable and entitled to credit, and who, being duly sworn, declare, in relation to the aforesaid case as follows: That he is well acquainted with John Brandon the above named soldier, and also acquainted with his physical condition. That he has well know his physical condition for one year last past. All of which time he has been suffering from lung disease. Which he understands to be result of gunshot wound through lung and body. From which disease he is now totally helpless and compelled to have the aid and assistance of other persons. Brandon aged 69 years a resident of No. _ Street, County of Saline, State of Illinois and . well known to me to be reputable and entitled to credit, and being duly sworn, declare, in relation to the aforesaid case as follows: That he has the means of knowing and does know the physical condition of John Brandon the above named soldier and that he has been suffering for many years last past from lung disease as result of gunshot wound through lung and body. That said disabilities have so increased that for the last six months Months (sic) said soldier has not been able to care for his person and has been compelled to have the aid and assistance of other persons. That he is the brother of said soldier and has kept him at his home in Stonefort, Ill for near three months last past. Where he has had him nursed and cared for until two weeks ago, at which time he had said soldier sent to his own home in the country. Where he is now compelled to have the constant attendance of other persons both day and night that his pension of $30 per month is wholly insufficient to pay for his medical and other attendance and leaves him nothing for support. Grace aged 58 years a resident of No. _ Street, County of Saline, State of Illinois and . well known to me to be reputable and entitled to credit, and being duly sworn, declare, in relation to the aforesaid case as follows: That he is well acquainted with John Brandon the above named soldier and has been for about 20 years, the greater portion of which time he has been said soldiers family physician. All of which time soldier has been suffering from disease lungs which he considers the after effects of a gunshot wound through his body and lung. That from the effect of said disease he is now so feeble that he is compelled to have the constant aide and assistance of other persons. That he is now soldiers attending physician. That he now considers send soldier beyond the reach of Medical and and (sic) without hope of recovery. Therefore reccommends the soldier be granted the highest rate of pension allowed by him in such case. That he has been a regular practicing physician for 25 years last past. Witnesses: "GENERAL AFFIDAVIT FOR TWO WITNESSES. State of Illinois, County of Saline, SS: In the matter of appeal in claim for increase pension No. 79.142 of John Brandon Co. "G" 31 Regt Ill Vols Infty. ON THIS 8 day of August, 1908 personally appeared before me, a Notary Public within and for the County and State aforesaid, duly authorized to administer oaths, Dr. B.F. St. John aged 73 years a resident of No. _ Street, County of Saline, State of Illinois and Dr. F.J. Osburn, aged 68 years, a resident of No. _ Street, County of Saline, State of Illinois, well known to me to be reputable and entitled to credit, and being duly sworn, declare, in relation to the aforesaid case as follows: That they and each of them are well acquainted with the above named soldier John Brandon and with his physical condition and have been acquainted for many years past. That they have the means of knowing and do know from frequent examinations and observations that said soldier has been and is now suffering from disease of lungs as result of gunshot wound through lung and body. For the last six months he has been gradually growing worse. That he is now compelled to have the constant (not periodical) and and (sic) attendance of other persons both day and night. That they give as their opinion professionally that he is now beyond the reach of Medical aid and without hope of recovery. That they have had favorable opportunities to observe soldier's condition for the last three months as he has been kept at this place (Stonefort) and cared for by his brother until about two weeks ago. At which time he was handed out to his home in the country. Therefore, they would recommend that he be allowed the highest rate of pension allowed by law in such cases. That they are regular practicing physicians and have been for more than 35 years last past. They certify that they are not interested in the prosecution of this case. Respectfully returned to the Commissioner of Pensions, with the information that in case of John Brandon Co. G, 31 Reg't Ills. Inf. in addition to the statement or statements herewith returned, the military records show the following: Age 18, height 5 feet, 6 inches, complexion light, eyes Gray, hair Dark, place of birth, Roan, Tenn. occupation Farmer, Found also as John Brandon Rolls from M.J. to April 30, 63 present. Aug. 31, 63 wounded and in Hospl St. Louis, absent. Oct. 31, 63 same. Dec. 31, 63 present. To June 30, 65 same. Prisoner of War recorder show him present at B.B. No. Aug. 26 and Aug. 31, 63. The medical records show him treated as follows: As John Brandon, Pvt. Co. G 31 Ill. Vols. Wounded Badly at the battle of dehamption Hill Miss May 16, 63; treated 6,63 to_, diagnosis not stated; June 8 to 13, 63 G.S. Wound Lt. side; June 14 to July 7, 63 Gun shot wounds Rt. Arm; July 6 to 9, 63 diagnosis not stated; July 9, 63 to vulnus sclopet; Nov. 4, 63 to _ vulnus punctum; Dec. 8, 63 to _ intermittent; as J. Brendan -65. Wound unfit for duty. The Medical records also show _ Brandon, Pvt. Co. G 31 Ill. Inf. treated Feb, 8 to 15, 63 Intermittent; Mch.1,63 deonstipotion; Mch. 7&8, 63 Intermittent; and Brandon, Corp &c, Jany 22, 64 Intermittent. There were two (2) men surnamed Brandon in Co. G. 31 Ill Inf. It cannot be determined to which of them this record relates. Nothing additional found. F.C. Ainsworth, The Adjutant General."
Letter: "[Form] 3-1891. (Initials in top corner, I.M.F., J.M.R., and S.W.B.) DEPARTMENT OF THE INTERIOR, BUREAU OF PENSIONS, WASHINGTON, D.C., September 14, 1908. -Med. Div.- Cert. No. 79.142, John Brandon, Co. G, 31 Ill., Vol. Inf. Mr. S.A. Cuddy, Chief of Law Division. Sir: Appeal is taken from the medical action dated July 15, 1908, denying increase from second grade for gunshot wound of left side and lung. As evidence has been filed since this action tending to show a material increase in the degree of disability, permission to order a test medical examination before answering the appeal is requested of the Hon. Returned for further Action and Report. The Commissioner of Pensions, Sir: In your report on the above-entitled claim you state, September 15, 1908, that, with approval of the Department, the action proposed by the Medical Referee will be taken. This officer, September 14, 1908, proposed for reasons stated, to direct a test medical examination relative to claimant's title to an increased rate before responding to the appeal. This proposed action meets the approval of the Department. The papers are herewith returned for the purpose indicated. After the examination and consequent readjudication, please return the papers with your further report in order that final disposition of the appeal may be made. Letter requesting Exam: "HOME. It is the practice of the Bureau not to order a surgeon to travel over thrity miles from his home to make an examination. If it is desired that a surgeon should travel a greater distance than thirty miles, that fact will appear in this order. If it is not clearly set forth in the order to the surgeon, he should notify this Bureau, that the order may be canceled, or the surgeon instructed to comply with its requirements. [Form] 3-133. Fern, Secretary Board of Examining Surgeons, Vienna, Ill. SIR: The applicant in the above- entitled claim for pension is said to be unable to report to your board for examination. As soon as possible, therefore, at a time which will not conflict with a meeting of your board, the member of your board residing nearest the claimant will proceed to the home of said claimant at Stone Fort, Saline Co., Ill. and examine him very carefully in accordance with the instructions inclosed, and then return to the Bureau this letter and the other papers herewith, closely observing the directions of Form 3-168. Instruction to Doctor: "Appeal Test Home. General Law, Western Division (initials MMG, JWB, DWB) Department of the Interior, BUREAU OF PENSIONS, Washington, D.C. SEP 20, 1908 Dr. W.J. Ferns, Secretary, Vienna, Co. Johnson, State Ill. Sir: Mr. John Brandon, P.O. Stone Fort., Co. Saline, State, Illinois. Late a private. Co. G. 31 Regiment Ill. Vol. Inf. and applicant for increase. Invalid Pension Ctf. No. 79.142. Home for examination on account of the disability or disabilities mentioned hereon. If other causes of disability are found on examination, they should be described. Evidences of the results of vicious habits should be sought in every case and reported on. Gunshot wound of left side and lung. The object of this examination is to learn the actual physical condition of the claimant and also the exact degree of disability from the pensioned cause. Please note and (if possible) real weight and height, and pulse and respiration rates while sitting, standing and after active exercise. Locate on the diagrams and fully describe all scars. Are they tender, adherent or dragging? What parts and tissues were injured? Report condition of heart and each lung as determined by inspection, palpation, mensuration, percussion and musculation of either lung or a cavity? If so, where? Does he have pulmonary hemorhages or severe night sweats? I dyshnoea very marked? Is there cardiac murmur? Note location and extent of any cyanosis or edema. State condition of throat and results of urinalysis. Is there any evidence of rheumatism or Iraralysis? Also, describe it fully. What senile changes do you observe? Has he cataract, paralysis, agitans or hypertrophy of prostate gland? Is he confined to his home or bed? If so, what keeps him there? Does he require aid and attendance of another person? If so, why, how often and for what purposes does he need help? Can he dress, feed himself, and walk? If not, why? Describe every disability. V. Warner, Commissioner. Marked across - Claimant Dead."
Final Doctor's Report: "Appeal Sec. Oct, 8, 1908, Dock. 125161. Papers returned by Dr. W.J. Fern, Secy, Board, Vienna, Ill with this information that soldier died about three weeks ago as reported by Wm. Jauner, P.M. in an undated indorsement record Oct. 15, 1908. Chief of Finance Division advised - WWA Oct 17, 1908. Medical Referee."

SIR: You are hereby directed to drop from the roll the name of the above-described pensioner who died Aug. 31, 1908. V. Warner, Commissioner. REPORT. Commissioner of Pensions. SIR: The name of the above-described pensioner, who was last paid at $30 per month to 4 July, 1908 has this day been dropped from the roll of this agency. Charles Bent, U.S. Pension Agent.
