Well known to me to Be reputable and entitled to credit and who being duly sworn Declares in relation to the aforesaid case as follows. That he is a practicing Physician and has been aquanted with the Above named soldier for about 15 years and has this day carefully Examined him find him suffering from rhumatism of the right side With tenderness over the lower border of ribs and sourness of Mussels of back and neck also injure of left ankle joint with Stiffness of said joint and soreness, fingers on right hand closed And partial ???????. Claimant also has piles with inflamation and Hemorhoids. In my opinion claimant is at least 3/4 disabled from the Preformance of manual labor by reason of said disability and that I Have no interest direct or indirect of the prosecution of this claim. H.G. That he was not employed in the military or naval service of the United States otherwise than as stated above.
