The following text is a transcript of a copy of the original pension request from John Gilchrist, for his disabilities received from injuries during the Civil War.

Declaration for an Original Invalid Pension

This must be executed before a Court of Record or some Office thereof having
Custody of the Seal.

Territory of Dakota, County of Lake, ss.
on this 12th day of March A.D. one thousand eight hundred and eighty three.
personally appeared before me Probate Judge of the Probate Court, a Court of
Record within and for the court and Territory aforesaid John Gilchrist, aged 53
years, who, being duly sworn according to the law, declares that he is the identical
John Gilchrist who was enrolled on the 19th day of August, 1862, in company "F"
of the 21st Regiment of Infantry, Wisconsin, commanded by Colonel Sweet, and
was honorably discharged at Milwaukee, Wisconsin on the 8th day of June, 1865;
that his personal description is as follows:  age 53 years; height 5 feet 11 inches;
complexion slight; hair, auburn; eyes, blue.  That while a member of the
organization aforesaid, in the service and in the line of his duty at Perryville in the
State of Kentucky, on or about the 8th day of October A.D. 1862, he - while his
company and regiment (just after having marched through a cornfield) were
forming in line of battle to attack the Rebel army - commanded by General Bragg -
was shot at by the said Rebels, and hit by one of their bullets, in the front of his left
leg, about 5 inches below his knee joint, which bullet went clear through his said leg
from it's effects of which he was made sick, and never has recovered.  That prior to
his enlistment he was sound and free from disease.  That in consequence of said
wound he lost much blood, was made very weak and sick and suffered much pain
ever since.  He was obliged to remain on the field of battle from the time he was
wounded until the next day at 10 o'clock A.M. and then was placed and remained in
hospital for a period of 5 months.  That in June 1864, at the battle of Pumpkin Vine
Creek, he was again wounded in his right wrist - that while in service he was
engaged in 12 battles and engagements.

That he was treated in hospital as follows:  From about Oct 8th, 1862 to January
17th, 1863, he was in Hospital #9 (?) at Louisville, Kentucky and from that time
until the 8th of March 1863, he was in hospital at Camp Davison, Ohio, and at both
places between said restrictive duties was under surgeon's care.  1st under Dr S.
Ballett (?), & 2nd under Dr. Avado (?) of Cincinnati Ohio, in all a period of five
months.

That he has not been employed in the military or naval service otherwise than as
stated above at any time prior or subsequent to the service mentioned first above.

That he has not been in the military or naval service for the United States since the
8th day of June 1865.

That since leaving the service this applicant has resided in the town of Springvale in
the State of Wisconsin up to September 1874 since which time he has resided in
Lake County, Dakota, and that his occupation has been that of a farmer.

That prior to his entry into the service above named, he was a man of good, sound,
physical health, being when enrolled a farmer.  That he is now nearly wholly
disabled from obtaining his subsistance by manual labor by reason of his injuries,
above described, received in the service of the United States; and he therefore
makes this declaration for the purpose of being placed on the invalid pension roll of
the United States.  He hereby appoints with full power of substitution and
revocation.

His true and lawful attorneys to prosecute his claim.  That he has never received nor
applied for a pension; that his residence is Madison, in Lake County, the Territory
of Dakota, and that his post office address is the same place.

Respectfully returned to the Commissioner of Pensions.

John Gilchrist, a private of Company 'F', 21st Regiment, Wisconsin Volunteers,
was enrolled on the 19th day of August, 1862, at Springvale, for three years and is
reported on Company rolls from September 5th, 1862 to December 31st, 1862. 
Absent, wounded at battle of Chaplin Heights, October 8th, 1862, left at Maxville,
KY, so reported for January & February 1863.  March & April 1863, present so
reported to August 31st, 1863, September & October 1863.  Absent    Detailed as
guard with supply  train to Stephenson, Alabama, October 24th 1863.  November &
December 1863, present so reported to April 30th, 1864.  May & June 1864 absent. 
Wounded near New Hope Church, Georgia, May 28th, 1864.  July 1864 present so
reported to April 30th 1865.

Mustered out with Company Jas (?) Corps, June 8th, 1865.

Company was in action October 8th, 1862 at Chaplin Hills, KY, and May 28th,
1864 at New Hope Church, Georgia.

Nature and location of wound October 8th, 1862 not shown.   List of Casualties of
Regiment. May 27th to June 5th, 1864 at Dallas, Georgia, shows him wounded, left
arm slightly.  Exact date not stated.  Another list May 3rd to September 6th of 1864,
shows him wounded right wrist.  Exact date not stated.  Another list shows him
wounded May 28th of 1864, New Hope Church, Georgia.  No additional
information related to wounds.  Hospital record of Regiment not on file.
