Street Address (at time of application) 3. Post Office 4. Name of present Employer 5. Address of Emloyer 6. Age at last birthday 7. Date of birth 8. Place of birth 9. Father's full name 10. Mother's Full Maiden Name 11. Sex 12. Color If registered with US Employment Service, give registration Number _____ If you have previously filled out a card like this, state place & date _____ Date signed Signature as usually written Keep in mind - Social Security has not always been around. It was started around 1932. Only men who worked for someone when Social Security was instituted, or after, got cards. Many farmers and self employed men had no need for them. Quite often older men, (past working age) would not have gotten them either. Women did not typically work outside the home until the 1960's. Many women never got Social Security cards, collecting only widow's benefits at the time of her husband's death. Women that DID work outside of the home, were also required to register with Social Security.
