Hi!
  You have to get an affidavide (sp) made to attach to the document. To get
it you have to get proof of your corrections. Such as I have to send to Los
Angeles and get my Grandmothers middle name which is a Scots last name
changed.  My mother in her grief of her fathers death has it wrong on his
death certificate.  You need to provide two if not three forms of proof I
would think. Also a living relative other than yourself could provide
confirmation. I would write first to the county clerk and ask for the proper
procedure.  
        My Dad's birth certificate had the first and middle name and luckily
his parents were still alive to sign the affidavide. Whoever wrote the name
switched his fathers name around and didn't even consider that his middle
name was his mother's maiden name.  Hope this helps, Linda in Albuq.
