A point I think some people may be missing.  The posting says this is a "document".  If it is the 
transcription of a legal document, the person may have transcribed exactly as written.  As you 
know, many documents contain mistakes just like those listed in the plea for advice. I have some 
legal papers with the same name written as many as four different ways within the same document.  
If this is the case, the document should be posted exactly as it was written.
