Notes: Primary Record Confederate States of America: I certify, that the within named Levi W. Goodan a private of Capt George W. Conners Co. of the _____of the State of Kentucky, age 55 years, 5 feet 10 inches high, fair complexion------eyes gray hair and by occupation farmer was enlisted by Major R. B. Thomas at Prestonburg on 21 Day Oct 1816, to serve one year and is now entitled to discharge by reason of extreme old age and bodily infirmity. The said L.W. Goodan was paid by Major Thos Fisher to include the 30th day of April 1862 and has pay due from that date to the present date. There is due to him $14.00 traveling allowance Camp Calfer ?, the place of discharge to Prestonburg the place of enrollment, transportation not being furnished in kind. There is due him -----. He is indebted to the Confederate State Thirty dollars on account of clothing. For pay from 30 of Apr 1862 to 31st of May 1862 being one month and ___days, at eleven dollars per month for pay for traveling from Camp Calfer to Prestonburg, being 140 miles at ten cents per mile. Deduct for clothing overdrawn. Amount due for clothing $20. Balance paid $45. Received of Major Thos Fisher Qr.
