The following account has been copied as accurately as possible from the original, which is written, in ink, on two sheets of paper, 7 ½" x 12", folded to make 4 pages, sewed in the center. This is then again folded over to make the final booklet 3 ¾ " x 6", for storage. On the outside of the 3 ¾" x 6" fold is written:
