There are two methods of submitting your information:

1. Your Name eg; Don & Terasa Hodges

2. Your Mailing Address eg; PO Box 1116 Independence, Kansas 67301 (this is the snail mail address!)

3. Your E:Mail address

4. Your Web Page address (URL)

5. Nature of the information available. Attach files to your e:mail if you wish but state the file-type in your message.

When writing your information, please don't use all CAPITALS - use capitals for SURNAMEs only.

It is important to provide your postal address because not everyone will have e:mail.
