On this 19 day of December, A.D., 1898 before the undersigned authority within and for the above county and state personally appeared Ed Tilley, well known to me to be reputable and entitled to credit, and who being duly sworn, declared in relation to the aforesaid case as follows:
I personly know Dennis C. Quigley from 1863. and before he went in to the army he was strong and healthy and I personly know him after he came back from the army and he was in bad health with lung truble and chronic diareah and rheumatism and I saw him at diferant times and he is still complaing of lung truble and rheumatism by coffin and swelling of the joints and stifness this is ritten by my own hand.

That I am a brother of Dennis C. Quigley. I personally know Dennis C. Quigley from a boy up until the fall of 1864. And up to that time he was a stout healthy young man. That I did not see him again until January 1866 then he was pore and weak looked at least 10 years older than when he went to the army. He complained at that time of Cronic dire (diarrhea?) Rheumatism and lung trouble.

Clark, aged 68 years, a resident of Cedar Rapids, in the County of Linn, and State of Iowa, well known to me to be reputable and entitled to credit, and who, being duly sworn, declared in relation to aforesaid case as follows:
I was Captain of Co. D, 12th Regt Iowa Inft. in 1865, and know that in Sept or Oct 1865 he - claimant, Dennis C. Quigley had an attack of measles, and was because of this sent to the hospital. I think Dr. Barr was the regimental physician or surgeon at that time. I know that on his return from the hospital he was troubled with chronic diarrhea and that he was suffering in same way the time of our discharge from the service at Davenport, Iowa.

His post-office address is Cedar Rapids, Iowa. He further declares that he has no interest in said case and is not concerned in its prosecution.

I can't give Doctor's evidence as I have not doctored with any Doctor since January first 1901. Why I didn't doctor was because I was short of means to doctor with.

Sir, I have per known Dennis C. Quigley since 1866 when he came out of the army. he was my next door neighbor. he has been troubled with rheumatism and lung trouble as the dockters has proven. he has no bad habits nor ever had any. he deserves a pension.

Sir, I have known Dennis C. Quigley ever since 1866 when he returned from Civil War. I lived in two mile of him and have known him ever since. I know he has been afflicted with lung trouble and rheumatism. If any one deserves a pension he does. He is an honorable and uprite man with no bad habits.

J.A. Hughes, aged 45 years, a resident of Mallard in the County of Palo Alto and State of Iowa whose Post-Office address is Mallard Iowa, well known to me to be reputable and entitled to credit, and who, being duly sworn declares in relation to aforesaid case as follows:
That we have been neighbors of Dennis C. Quigley above named for over seven years. That he has complained of Rheumatism and Lung trouble more or less during this whole period. That his joints have been badly swolen and he has a cough every since we knew him up to the present date. That we know him to be a temperate man and that the above troubles have not been caused from vicious habits. We know that since the 3rd day of January 1901, he has not been able to do more than one half a mans work.

Their postoffice address is Mallard, Iowa. They further declare that they have no interest in said case and are not concerned with its prosecution.

Application for Invalid Pension

On this 11th day of April, A.D., one thousand nine hundred and 13 personally appeared before me, a Notary Public within and for the County and State aforesaid, Dennis C. Quigley, who being duly sworn, according to law, declares that he is 70 years of age, and a resident of Donnybrook, County of Ward, State of North Dakota, and that he is the identical person who was enrolled at Davenport, Iowa under the name of Dennis C. Quigley, on the 17th day of January 1865, as a Private in Co. "D" 27th Regiment and Co. "D" 12th Regiment, Iowa., in the service of the United States, in the war of the rebellion, and served at least ninety days and was Honorably Discharged at Memphis, Tenn., on the 20th day of January 1866. That he was not employed in the military or naval service prior to January 17th, 1865. That he has not been employed in the military or naval service sine January 20th, 1866. That he was born on the 5th day of March, in the year 1843, at Maple Grove, Edwards County, Illinois. That he is a pensioner. That he has heretofore applied for pension. Certificate #1022150. That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the Act of May 11, 1912. That his post-office address is Donnybrook, County of Ward, State of North Dakota.

Mary E. Lytle being first duly sworn deposes and says that she is well acquainted with Dennis C. Quigley; that she is the Daughter of Dennis C. and Harriet Louise Quigley; that for the past year she has had the care of the household and sleeping apartment of said Dennis C. Quigley; that he is feeble and badly afflicted with Palsey, but is able to feed and dress himself, save that he requires aid in putting on heavy top coat or other heavy wearing apparel; that he is able to attend to all natures calls unaided; that he is not confined to his bed; that on the 18th day of November A. D., 1921, he was in the same condition as at this date and as set forth herein; that his mind was clear and keen and normal; that barring any unforeseen accident, it is safe for him to step out unattended; that no attending physician is had as the condition of Dennis C. Quigley is just such as would be expected from one afflicted with Palsey, and no medical aid can be had.

On this 2nd day of Feb., 1926, personally appeared before me, a Notary Public within and for the county and State aforesaid, Dennis C. Quigley, who being duly sworn according to law, declares that he is 83 years of age, and a resident of Kenmare, County of Ward, State of North Dakota; and that he is the identical person who enrolled at Roth Iowa, under the name of Dennis C. Quigley, on the 16th day of January, 1865, as a private, in Company D., 27th Iowa in the service of the United States, in the Civil War and was Honorably Discharged at Memphis, Tenn, on the 20th day of January 1866. That he also served (here give a complete statement of all other services , if any)________.
That his personal description at enlistment was as follows: Height 5 feet 9 1/2 inches; complexion Dark; color of eyes Hazel; color of hair Dark; that his occupation was Farmer; that he was born March 5th 1843 at Maple Grove Edwards County, Ill. That he requires the regular personal aid and attendance of another person on account of the following disabilities: Chronic arthritis, leg and arm, hypertrophy prostrate quite marked, Palsy very marked, Chronic Broncitis.
That since leaving the service he has resided at Pocahontas County Iowa, Stearnes County Minnesota, Palo Alto County Iowa, and Ward County N. Dak., and his occupation has been Farmer and Mail carrier. That he has applied for pension under original no. 1022150. That he is a pensioner under Certificate No. 1022150.
That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the Act of May 1, 1920.

Dr. D. A. Fisk, being first duly sworn deposes and says: that he makes this affidavit on behalf of Dennis C. Quigley for use in obtaining a larger award or pension from the U.S. Government; that he is a physician and has been duly licensed to practice medicine in the State of North Dakota for a period of fifteen years; that during that time and for the past five years has been the physician taking care of Dennis C. Quigley, IC 1022150 - D 27 Iowa Infantry, and that he knows the said Dennis C. Quigley to be totally disabled; that he is unable to care for himself and that at this time he has, and it is necessary that he have, a personal attendant; that my examinations have disclosed that he has very high blood pressure, arterio-sclerosis, he has a very bad heart, mitral regurgitation, he also is suffering from a chronic nephritis. Also, on September 1st, last, he fell and injured his left shoulder and owing to his advanced age he is crippled from that; he is in a very feeble condition at the present time. He also has chronic bronchitis.
